2024-2025 Registration Requirements

Items needed for Registration:

• If you are transferring from a Public School, please provide the following;

  1. Proof of address in parent/legal guardian’s name: FPL bill, MDWAS, Food Stamp letter, or SSI benefits letter

  2. Current Driver’s License or State ID with picture
      

• If you are transferring from another county, state, or country,  please provide the following;

  1. ​ Items 1 & 2 from above

  2. Birth Certificate

  3. Immunization and Physical (forms #DH680 and DH3040)

  4.  Official Transcript/Last report card
      

• If you are not the parent/guardian of the student and you have temporary guardianship, you must present custody documentation.

• If you have any questions, please email admissions@careerprepworkforce.org card & current report card received from the previous school.

• All Assessments & Placement Test completely filled out (example FAS, SAT, FAIR testing scores).

• A copy of the student's health and shot records (blue & yellow form).

• A copy of student birth certificate

• A copy of student current IEP (if student is ESE program).

• Copy of student’s Psychological Evaluation.

• A copy of parent/guardian photo ID and SS Card
